Paper files live where they were written. If the folder is at another facility, in a locked office, or lost to flooding, the clinician in front of the patient has nothing to read. Staff search shelves while a queue builds outside the door.

What replaces it, step by step

Register once, keep the identity
The front desk registers the patient once. The record carries a hospital number, a geocode down to the boma, and optionally a QR card or fingerprint, so the same person is found again without a folder.
Find the patient in seconds
Search by any of those identifiers. The result is the whole record: every past encounter, prescription, result and referral attached to that person.
Hold it on the device
The record lives on the facility device, so a power cut or a dropped connection does not remove access to the history in front of the clinician.
Replicate when a connection appears
When bandwidth returns, changes sync both ways. The file is no longer in one building on one shelf: it exists wherever that patient presents.
